Nope, don't believe there are any online. I've seen different forms used for this, just depends on the court. In my court, you fill out a motion on custody and you have to go down to the county clerk's office and file it in person, you notify them that you want an ex parte hearing. They take the form to a Judge(not always your Judge)who looks it over and decides if the matters needs an emergency hearing. In a different court then mine, they had an actual form that said ex parte motion, but once you filled it out and handed it into the clerk, they again took that form to a Judge to decide if the matter needed an emergency hearing.
Be prepared because I have seen them hold the hearing same day you file, if they feel it is an emergency. If they feel it is not an emergency then the form is filed and they give you a court date. Time frame for non emergency hearing just depends on the court. You then follow the courts procedure on notifying the other parent.
Emergency hearings are very hard to come by though. One reason is many Mi courts move fast to begin with, so they find little that is a true emergency. Sometimes you really have to push to get them to have a Judge look at it, they often tell you right out you won't get an emergency hearing. However be nice, patient with them and push that you would like it considered by a Judge anyways.
The only time I have seen them do an emergency hearing, is when there is abuse and proof of that abuse or when one party does not return the child to the CP. I've seen a few when the CP went to jail and the NCP filed for custody, but even then they don't often see that as an emergency, specially if the child/children are with a 3rd party relative.
You can call FOC or the county clerk's office of the court that the case is in and ask if that court has a specific ex parte form or if motion on custody form is what you need. This would not be considered legal advice, they are allowed to tell you what form to use, but often you have to push this to.
