What is the name of your state (only U.S. law)? Florida

It has been almost three and a half years since the culmination of years of reckless behavior caused me to re-evaluate my life and begin the life of a recovering alcoholic. Things have done a 180 since those days, and wonderful things have happened that I never could have dreamed. Well, my wife and I have decided that it is time for me to get my license (hardship) back and that is the reason I created this thread. I have 2 DUI's. I refused to submit to a breathalyzer the second time and consequently had my license suspended. While on suspension the incident that put an end to my insanity took place. So as not to bore anyone who reads this, the outcome of that incident was "driving on a suspended license with knowledge" and "running a red light". I was also uninsured at this point. My wife and I were married June 26, 2010 and recently State Farm informed her that they were dropping her due to my record. In the letter they sent they listed some of my record as the reason they were dropping her. Those reasons were:
-Refusing breath test - License suspended
-DUI - License Revoked
-Habitual Traffic Offender (Driving w/ License Suspended, 3rd major offense in 3 years)
-Financial responsibility suspension without auto insurance

Now I know my license was revoked for 5 years due to the 2nd DUI, what I don't know is what has happened due to the other charges. I was never informed of anything happening as a result of the latter charges. Is my license suspended for longer? Was there a 2nd five year suspension but will they run concurrently? I am trying to get my hardship license and I guess I am just nervous / anxious about it. Any help shedding light on my situation will be greatly appreciated. Thank you
