What is the name of your state? Florida

Due to another tremendous increase in our
Group Health Insurance, our employees have
all chosen to cancel coverages under our Group
Health Insurance planand be responsible
for their own, if desired.

I need to locate a general form which I can have
all employees sign, acknowledging their request
to cancel and assuming all responsibilities for any
health insurance benefits they may or may not
choose.

**A: and where is your company attorney? Did you bother to check your state regulations to see if there is mandatory health insurance required by employers?

GROUP HEALTH PLAN

No, there is NO mandatory Health Insurance requirement
in the State of Florida.

I have not contacted our Corp. attorney as I would just as
not spend what would probably amount to $200.00 for the
consult and another $300.00 + $500.00 to have the form drawn
up. I do use our Corp. attorney but only in matters which are
obvious that I can't handle.

I'm a Northern Girl
The only state where there is mandatory health insurance is Hawaii, and even then it's only for employers of a certain size.

Sorry, I don't have the kind of form you're looking for, but I don't see why it's necessary. Cancel the insurance, send around a memo that the insurance will no longer be effective as of such and such a date (check with your state insurance commission to be certain of any mandatory notice periods) and then they're on their own.
