H4 Visa for Child

My friend and his wife stayed in US on valid H1B Visas. Last year she got pregnant in US but she never consulted doctor in US. During 30 weeks of pregnancy, she travelled to India and in 33 weeks she delivered premature baby in India. Baby is five months now. She has Birth of certificate and passport for her child. She wants to come to US on H4 Visa along with child. Her H4 visa is approved. Now she is planning to apply for H4 visa for her baby.

1) Is there any chance of H4 visa rejection for child as mother never visted doctor in USA?
2) For her five months baby, can she apply H4 visa via Dropbox in India?
3) Can a consulate raise a question on her belief not to visit the doctor during her pregnancy?

The only issue is "it is her child" and the child is qualified with regard to whoever the H1 holder is. Where it was born or what the care conditions were don't much matter.
