Having problems with the seller and the title

I bought a car from a person that I work with. I paid cash for it, and when I bought it, I received a bill of sale from the person I bought it from. He couldn't find the title, and he told me he would get it to me the next day.
The next day, he gives me an "Application for Duplicate Title with Transfer", which I believe is perfectly fine with the DMV. However, there was a section where he had to have it notarized, and he didn't have it done. I gave him the paper back and told him to have it notarized, and he agreed. That was a week ago, and every day he keeps giving me excuses of why he hasn't had it notarized. Now, he has also disconnected his cell phone number so the only contact I have with him is through work. What can I do? (State of California, Los Angeles County)
