Health Insurance premium not paid by employer

UTAH -My husband has been paying insurance premiums to through his employer by having money withheld from his paychecks every 2 weeks. His employer decides to switch insurance companies and we find out that we didn't have any insurance coverage for 15 days with no insurance company but we had paid our premiums for that period. We have since left that company, but are trying to get bills paid that we had incurred during those 15 days of no insurance. The employer informs us that the company will pay for those claims out of pocket. Is this legal, since we paid for coverage for the health insurance we choose to participate. Doesn't the employer have to notify us, if we choose to participate in this self -funded plan, and is self funded employer coverage even legal for only 15 days.
