health insurance with a large company

after being employed by a large corp for the last year and six months my health, dental,and perscription insurance because of a one dollar and fifty eight cents shortage in my payroll. #1 i need thirty two hours as aserver to maintain my insurance which this last winter i was not given. i got my hours back and was able to catch up. then i get another letter telling me i am behind again. i call the home office in tn and am told i do not owe 200.00+dollars that they figure 3 weeks of payments together and so on . they can not
