What is the name of your state (only U.S. law)? Minnesota

I quit my job on Sept 12. I paid my weekly health insurance premium on Sept. 12 which covers me for the week of Sept 15 thru sept 21. On Sept 15 my employer canceled my health insurance. Am I entitled to a refund for paying for health insurance I did not receive?

Not necessarily. Do you know for an absolute fact that insurance paid on 9-12 goes to cover 9-15 to 9-21 and not 9-8 through 9-14? There is no law that requires that it be paid forward and many companies do, in fact, have it pay backwards.

You need to find out from your employer. The insurance carrier has no clue how the payments you make are being applied.
