I am a Paramedic and we work medical coverage for sports events. A lot of the events have forms that they want filled out, if there is a injury. Most want Name,Address,DOB, and description of injury.

Is this a HIPAA violation to provide this information to the event sponsor?

Yes it is, and busy-bodies are ubiquitously dangerous who "Think they know, but they don't know." Chances are they wouldn't know what to do with such information even if you were unmindful enough to purloin it for them.​

Private Healthcare information such as mentioned is "For Your Eyes Only" to healthcare providers, unless a patient signs a disclosure of medical information to a specific party like "The Chicago National League Ball Club".​

This is one of the reasons you hold an EMT license with a serial number: it allows you certain critical info important to the life of those your care reaches at events, and as such, football players are not licensed EMT's, are not privvy to it. :eek:
