The easiest way to do it is to hire an attorney and let them handle it.

Barring that, you will need to find the appropriate paperwork for your state and/or county, take it down to the courthouse, pay the filing fee, have the clerk file it for you, have the motion served to the other party and wait for a court date to come around.
