I devloped a database for a company they asked if they could make payments to me and I agreed but to this date they have not made any payments to me. They were to start paying me on the 25th of September. I sent a letter asking for payment now I want to know is there some type of leagal form I can send them that will show them intent to take them to court.

Yes. Contact the Small Claims court in the county where they are located and get a complaint form. Fill it out and return it to the court with the appropriate fees. Then, the court will make sure that the company (now called the Defendant) gets notification of your action.
