You don't. There's no such thing as putting you on a deed. Real estate titles is not like some sort of club membership, nor necessarily good estate planning. Each deed conveys some ownership rights from one person to another. If grandmother wants to give you all or part ownership in the house, she should consult an attorney who can explain to her why that may not be a good idea and if she insists, prepare and file the deed. If grandmother doesn't want to unconditionally give you ownership of all or part of her house, she should not deed anything.

you can also buy her out via a normal mortgage or if she agreed to sell it to you at a fair price via contract for deed that's another way since any so called deals should be at a fair price when selling to a relative then later if she needed nursing home care or assistance from the state they cant claim property was sold too cheaply as far as so called govt sourced benefits went.

There are a few other reasons why it's a bad deal for grandma. If she ever needs a reverse mortgage, having partial ownership by someone, not 65 will bar her from getting that. As stated, giving away assets can screw her Medicaid eligibility. If she ever needs the money, the fact that there is now some other owner on it complicates the problem.

It's potentially a bad deal for the granddaughter as you pick up grandma's presumably low basis on the share deeded.
