I am looking to get information to see if I can move my father in-law from Kentucky to California. He lived in California prior to moving back east in 1993. He is currently in a care home and on Medicaid or permanent disability. His wages were earned in California and the majority of his family lives in California. I need someone to point me in a direction where I can see if he is allowed to come back to California in a care home and have the same coverage and how to go about moving him. If someone could please point me in the right direction that would be great. Thanks in advance for any and all help/advice on experiences and agencies to contact.

There are several things you can do.
Contact your local CFILC Independent Living Center, they will have knowledgable people on staff who can provide guidance re disability issues and some insight into the working of accessing services here is a Directory

Contact your county Adult Protective Services or Adult services through social services who can inform you what requirements exist in your county, call different counties if several are connsidered. This may also include referrals to providers, assessment of eligible services and or application for SSI.

He can move, but chances are he, or you, will bear the cost of moving him. The tricky part is to get the Medicaid switched to Medi-Cal, because one must be a resident of the state of California before qualifying.

That means that Kentucky Medicaid will have to agree to cover him until his California Medi-cal kicks in. His current nursing home has a social worker on staff. This person should be able to help you figure out what you need to do at that end.

For California Medi-Cal, you can get an application online here: medi-cal.ca.gov

As I said, you will probably have to pay for his transportation, and depending on his physical condition and limitations, that can be costly.
