How to determine ownership % in a corp.

I have filed for my permanent residency with INS through employment, and in the final stage they have requested further evidence for determining ownership of my employer to make sure I don't have a significant stake in the organization. The exact wording is

"submit evidence of the ownership of the petitioning employer. Indicate each owner and their percentage of ownership. Additionally indicate any familial relationship you have to any owner(s)."

My employer is a private corporation and the HR is not aware of what would constitute as evidence of ownership. I guess revealing tax filings is not a possibility. Is there any other form or filing that might have ownership mentioned? The corporation is located in Ohio. Thanks for the help

You use the Articles of Incorporation and any later changes reflecting in BOD meeting minutes to the initial stock ownership.
