How to dissolve a partnership

What is the name of your state? Maryland

I was co-owner of a photography studio. My business partner told me a week ago she wants out of the business. She wanted just a few items and was leaving the rest. She said I could keep the company name, and all it's contracts, clients, bank accounts etc. She agreed to freelance on the wedding contracts we currently have booked.

I would like to get some kind of formal/legal agreement written up that makes the dissolve of the partnership official. I would like to make sure that in the future she can not come back and claim any ownership to earnings or assets.

How can I do this? Do I need a lawyer to write this up or is there a form I can download online?
