Inconsistent name in records

What is the name of your state? California

My name appears differently on almost all of my credit cards, mortgage, drivers license, SS, school records, etc.

First name = O
Middle name = H
Mom's maiden name = V
Last name = P

And I'm a Jr.

These variations are on my records: OHP, OVP, Jr., OVP, OP, Jr... etc.

What is the best way to make them all consistent? Will the standard name change process be adequate to note all of these variations?

BTW, the name on my birth certificate and SS are the same... OHP.

This is a common problem - particularly among cultures where last names include hyphenated names deriving from maternal names, etc. (Mexican names in particular). This is why so much stock is held in ID cards and social security numbers as a means to help identify the individual.

There will be no easy way to make all these changes. In fact, I doubt it will be possible - hence I would just make sure the key ones are consistent. Good luck in trying, though.
