What is the name of your state? NY

We never had authorization forms to sign to withdraw medical deductions from our paycheck. We only had a memo from the company stating what they will contribute. However, if you completed a HMO form is that giving them authority to withdraw the medical deduction from your paycheck?

It's a small company. I've worked at other companies that we completed an enrollment form with the deduction listed and had to sign it.

Any input will help.

If you have been notified that the employee does not pay the entire amount of the insurance premium, by completing the enrollment form you are agreeing to have your share of the premium deducted.
