What is the name of your state? MO
School started on Thursday. I put a note in the 1st grader's backpack for her principal, outlining the bus schedule (so they get on the right bus to go to daycare on my days). I included an identical note in the 4th grader's packet of forms that we received Tues night at her orientation.
Neither note actually made it to either school. The kids told me that SMom pulled them out and gave them to their dad.
I went through the 1st Grader's packet last night since it was due back today, and the doctor that's listed on the medical form is NOT the pediatrician that the kids have attended for years. It's the pediatrician that I told EX I did NOT want them transferring to.
There's a space for 3 emergency contacts (other than the parents). 2 of the contacts that he filled in are people I don't know, so I must assume they have no way of contacting me. The third contact was my father, but there was no phone number for him listed. I KNOW ex has his number. On the space for the parent's info, EX filled in his information, listed my first name only, and included no phone numbers - again, I KNOW he has them - and under 'place of employment' he filled in none. He KNOWS I'm working again, and he has the name, address and phone number of the company.
He listed SMom as babysitter, with her information, and listed his house as the only authorized bus stop for both before and after school.
He put on the medical form that the 1st grader is allergic to penicillan. This is NOT the case. The 4th grader is allergic.
I filled out all of the 4th grader's paperwork, using the emergency #s we've always used - my father and 2 of my sisters. I listed the pediatrician that they've always gone to. I, of course, filled in that she's allergic to penicillan. And, when I filled out the babysitter information, I included both the daycare that I'm using for my days, and his wife for his days. I listed phone numbers for both.
I sent a note to the 1st grader's principal today, letting her know the bus schedule, and I've called and left a message with her as well. I also asked her to send home new medical forms and emergency contact phones.
I'm not screwing myself in any way by filling out new copies of the forms that he filled out, am I? I'm keeping the ones he filled out that have so much missing and incorrect information.
Help?
**Note: Even though I filled out the 4th grader's information, I have no idea if it arrived at the school with the same information on it, or if it was changed. I'm calling that school too.**
