What is the name of your state? louisiana in 2001 my health insurance company went bankrupt and sent letters to all parties(group members and providers) they did business with..my problem is that a provider sent me a bill for services dating back to 1999 that the insurance company supposedly did not pay...am i entitled to documents stating whether the provider made an attempt to collect(deadline was sept. 2001), and, if so, am i held responsible for the amount owed?

You have the ultimate responsibility for any medical bills you incur, whether covered by insurance or not. The insurance company has no direct contractual obligation with the medical service provider.... and pays the bills on your behalf when covered.

One thing though.... you say the bill was incurred in 1999. Louisiana has a 10 year SOL for written contracts and a 3 year for non-written.
I suggest you contact the provider and ask them to validate the debt. Though they have no legal oblgation to do so (the FDCPA only applies to 3rd party collectors), most creditors will validate when requested.
