What is the name of your state?Wisconsin

My mother is very, very ill and may not make it past one more year. :( She owns her home and wants to transfer the deed to my name before she dies. Her fear is that hospitals may try to seize the residence because of unpaid bills. If we transfer the deed to my name (her only heir) while she is still alive, could we get into trouble?

Also, I have a Wisconsin Real Estate Transfer Return and a blank Warranty Deed to fill out to have this property switched to my name. Are these the proper forms, or is there anything else I need to accomplish this? Any advice on the best way to do this and help with the forms would be appreciated.

BTW, in case it matters, I am on the current HELOC that we have against the home, and this is now my primary residence. I had to move in to care for mother earlier this year. Thanks for any help!
