This happened in IL..I have a book wrote on the medical malpractice site. I think this is different thought.

I have had to request my medical records 3 times. Each time pages were missing ( example pg 1-5/ pg 3 would be missing.

All three times the medical records have had different sheets.

This is what I need to know. Is this Civil or Criminal?...How should I handle this?

I had my discharge sheets - On the third set of records I received today - One sheet happened to be in their that stated I could not sue the hospital, doctor, etc. It is clearly NOT my signature. The even left the "e" out of my last name.

Please help...I am at my witts end

Not criminal or even civil. Just a mistake (unless they are intentionally removing pages to hide things from you). Medical records retrieval, even in these days of computers, is not an exact science. Repeat the request for the full document. Refuse to pay a second time if there was a fee involved.

That may be a criminal issue. You haven't been damaged by the forgery. If you want to sue them, you can do so presenting the evidence that the signature on the waiver is forged.

On the medical records I received that does NOT have my signature on 2 of the sheets.

I live in MO....this happened in IL

Do I have to hire an attorney?...Or do I contact the prosacuting attorney's office?

Do I do this in MO or IL

It is the holiday's and I can't get in touch with anyone so hoping someone can tell me what to do about this.
