If someone has a court judgment against them, are their real estate holdings automatically affected, or must there be a order by the court to affect real estate holdings?

You have to place a lien on the property. Go here for more info: http://caag.state.ca.us/piu/small.htm It's written for small claims judgments, but the law is probably the same for larger judgments. It will point you to sources in CA.
