Usually you file suit, through a lawyer, in the state in which you live, provided the insurance company transacts business in that state. If you choose a distant location, without a good basis for doing so, they'd make mincemeat out of you and cause you to run up costs you can't believe. Most states have crowded courts, and they are not anxious to have out-of-staters crowd their dockets without a compelling need to sue in that state.

If this involves an employee benefit plan, and you are seeking to sue, there are lots of things you need to do first under ERISA.
