My response:
Since the document cannot be inspected, I can only offer you a conjecture to your question.
It appears to me from your description that his Mother was the "primary" beneficiary, and that you and daughter, together, were "secondary" beneficiaries, that would take from the proceeds of the policy only if his mother predeceased your husband, her son.
Since Mother was still alive, the insurance company only had a duty, by contract, with her, and not everyone in the world. So, despite the fact that you and daughter may have been named, they had no more duty to divulge information to you than, say, to me, or anyone else. Only Mother was in "privity" of contract with the insurance company.
