life insurance beneficiaries?

My mother-in-law's husband recently died. She was sent a form to sign that releases a small life insurance payment from the union he once worked for. The money is to be released to his grown children as he chose them as beneficiaries prior to marrying my mother-in-law. She was never given the oppurtunity to be designated as a beneficiary. Does the current spouse have any rights to this benefit? Do the kids automatically receive it? What if she doesn't sign the form to release the money?
