Lot/Plan Reference In Deed Is Wrong

What is the name of your state (only U.S. law)? Massachusetts

Is there an easy/inexpensive way to change the lot/plan reference in a deed if it is wrong? A revised lot/plan was approved but is not referenced in the deed. The deed refers to a previously approved lot/plan. There is no impact on neighbors lot lines because their deeds refer to the correct lot/plan for their property. There is a small piece of land that is in limbo between the two lots that currently does not belong to anyone due to the error. Since the banks lawyers did not find this during title search should they fix at no charge even though the original bank no longer has an interest in the property? Is something like this covered by title insurance?
