Married but giving my daughter "my" last name

When my new baby is born am I free to give her my last name (which is my "maiden name" cause I didn't change it when we got married) instead of my husbands? My husband will not contest this in any way. We made the decision years ago that all the children that come out of my body will have my name.

Will my husband or I need to fill out any special paperwork or anything or do we simply tell the staff at the hospital that my daughters name is to be the same as mine?

Just put her name as you want it on her birth certificate.
