What is the name of your state (only U.S. law)? TX

About 3 years ago, I was treated in a local hospital ER. I provided my health insurance information to the admissions people. They sent me a bill for my co-pay, which I paid.

Recently, I reviewed my credit report and found that I have 1 account in collections. It came as quite a shock as I've never had any account sent to collections and have excellent credit.

I called the collection agency and they told me it was the ER Physicians' Group. Total of about $400.

First, I never received a bill from this Physicians' Group. I presumed that they billed my insurance, just as the hospital did.

Second, I have never received any letters or phone calls from the collection agency. (I know that's incredible or incredulous :))

Do I have any legal means of getting this debt terminated?
