What is the name of your state? Missouri
My husband had some tests run in 2002. The claim was incorrectly filed. I sent our insurance company all the paperwork they asked for in order to get this straightened out. I didn't hear anything for 2 years. Now they have sent this in to a collection agency. We have switched insurance companies since this time. They keep telling me to have them submit the claim to them, but noone seems willing to do this. My question is can the collection agency freeze my bank account for a medical bill that is less that $1100?
Thanks for any advice.

Not unlesss they go thru the legal process of suing you and then winning a judgment against you. Unless or until they do that, all they can do is make threats.

You can contact the provider directly, ask them for a copy of the claim form with all the applicable diagnosis, cpt/hcpcs (pronounced HICPICS)/Revenue codes on a HCFA (HICFA) 1500 form/UB92, then you can submit a copy yourself to the insurance company.

Also, if the provider is contracted with the insurance and the PROVIDER was in error when filing the claim, they may be in breach of their contractual obligations and may be prohibited for billing you. But, since it looks like the insurer was seeking information from you, you may be at fault.

Contact your insurance company again, push for a manager and get them to help you. Ask them to reprocess the claim (they should have it since you state it was filed incorrectly), or ask them to contact the provider on your behalf. In my experience, that has the most chance of success.
