What is the name of your state (only U.S. law)?

Part of the application involve family history, my father's medical history is rather clear and he is still alive. My mother died decades ago, but she had a habit of keeping stuff to herself, often refusing to go to the doctor and often self-treated herself using herbal medicines. My father indicated that she had a minor heart attack at one point, but cannot remember if it was before or after she was 60. What should I put on the application if I want it to be truthful but at the same time not dispose me to a higher rate? Can I just say "minor heart attack at around 60 years of age"
