Medical Insurance NOT paying

I have a medical bill that should have been paid by my medical insurance years ago. I have sent many letters to the hospital and the medical insurance company for more information with NO reply. I want to know why this is NOT being paid by my medical insurance company. Why I can NOT get any answer from the hospital except a billing statement of pay up or else.
