Medical records and malpractice

So I have found myself in several situations where medical records have disappeared and wasn't allowed to pursue medical treatment.

There has also been situations where medical procedures have gone wrong and caused disfiguration and growth problems.

I would also like to discuss an instance of malpractice within my life and to seek legal advice for myself. I was under the age of eighteen and had been subject to medical malpractice within a pediatric office at a young age.
