What is the name of your state (only U.S. law)?
Texas

I received paperwork from the ex requesting that I sign a "waiver of citation" regarding his desire to obtain a passport for our 10 yr. old. He wants to have "superior possession" of the passport. In addition, he wants to modify the original decree and to request for two weeks of summer vacation. The paperwork was handed to me. I am not certain of why a waiver is necessary in order to obtain a passport nor why the decree needs to be changed. The original decree was mediated and to my understanding, any changes in it in the future were to be mediated as well. What is my best route in handling this? Do I have to make these changes?

No, you do not have to automatically agree to anything. However, whether or not you should agree depends on the circumstances.

For example, unless you have valid reason to believe that he would flee the country with the child, and could convince a judge of that, its likely that you will be ordered to cooperate with getting the child a passport. If he currently does not have at least two weeks uninterrupted time with the child, sometimes during the year, he is likely to get that as well...depending on the backstory.

Its a toss up as to who would get to hold the passport...and depending on the backstory. Most likely the judge is going to send you to mediation anyway.
