Mom won't read the CO, violating left and right

Dad has primary placement, custody is joint. Mom lives three hours away.

Mom was supposed to make calendar highlighting her placement days as set forth in the CO. She refused, so Dad made one. Mom refuses to acknowledge it.

Mom messes up every non-ordinary exchange, because she says she "doesn't understand the CO." Placement calendar that Dad made is a yearly calendar with highlighted days and attached list of dates and times of exchanges.

Mom just had son for her half of winter break. She went to the exchange place tonight, the wrong night. Her boyfriend called Dad (me) to tell me that she was waiting. I had to relay messages through the BF to Mom. Mom returned to her town and called me yelling that I was wrong because I told her the wrong day. The court order clearly specifies the vacation split and it is clearly spelled out on the placement calendar. I didn't "tell" her anything.

She is now refusing to meet me tomorrow, demanding that I drive 3 hours to her town to pick up my son.

There is already a contempt motion in the works for numerous other violations. Should I drive to the exchange point tomorrow and when she doesn't show file some sort of police report or something? Or should I drive all the way to her town and hope that she's there?

Can I add this to the motion? This happens all the time (because she "doesn't understand" the Order) and I'd like there to be some sort of repercussions that will get her to read the Order - or have someone else read and explain it to her.

You should inform her you will be at the exchange point. Then drive to the exchange point. From the exchange point -- if she is NOT there, call her and inform her that she is in contempt and she needs to come to the exchange point. If she refuses, go and get your son. Filing contempt is the only method you have of enforcing the court order. Filing a police report is proof that you were at the exchange point.
