What is the name of your state (only U.S. law)? Missouri
I have an issue and i need to know what recourse i have against the judge if any. My divorce was finalized in june of '11 my ex did not show to court resulting a default he had a year to come back which he did and have it set aside. The only reason he won the motion was because the judges office lost his acknowledgment of service. I need to know if i have grounds to 1. hold the judge and her office responsible for their negligence in losing the form and 2. to appeal the judges decision because it is docketed in the file in the judge hand writing the day it was received.

You mean, he didn't lose when you showed your copy of the form with the court's markings that it had been filed?
