My lawyer is not communicating!

My lawyer did not represent me well for the motions hearing for temporary orders in my divorce case. She did not review the documents she asked me to fill out and it turned out I did not fill them out correctly, which did not bode well for me at the hearing. I explained several points to my lawyer in response to my wife's statement, but she did not communicate any of these to the court, including my concern that my wife is negligent and abusive. She even miscalculated my earnings upward when filling out the child support worksheet. She did not give me a copy of the temporary orders after the hearing, I had to go to the court and print my own copies. She has not contacted me since the motions hearing, even though I have left several messages on voice mail, email, and with office staff.

I was told by a court clerk that I can not get a new lawyer until she removes herself from the case. In my last email I asked her to do this but she has not responded. She is clearly not representing me well and I am very concerned.

Is there some way for me to get rid of her?

Is there a proper forum/process where I can make a formal and public complaint with regards to her behavior?
