My response:
It shouldn't cost you one penny.
You see, on the Petition form is a box you could have checked that asks, "Do you want to revert to your maiden name?" or something similar. If that box wasn't checked, all you need to do is file an amendment to the Petition to request that the court should, as part of it's final orders, to allow you to return to your maiden name.
A request for restoration of either party's former name need not be made in the initial petition (or response). Upon request of a party in a marriage dissolution or nullity proceeding (but not in a legal separation proceeding), the court "shall" restore that party's birth name or former name. The request may be made ex parte, either before or after judgment.
