Name on house

My husband and I bought our house a few months before we were married. Because of this, my name is not on either the house nor the mortgage papers. How do we go about putting my name on the house?

I'm sorry, perhaps I'm confused or didn't clearly state my question. We are trying to 'add' my name to the house, not remove his. My understanding is that a "Quit Claim Deed" transferred ownership 'from' one person 'to' another. If you could clarify your answer, I would greatly appreciate your further help.
