Need an interrogatory form requesting insurance policy

We have a suit filed regarding an auto accident but, due to a conflict of interest, are interviewing new law firms. Previous attorney had not yet obtained a copy of the plaintiffs auto insurance policy so I must send an appropriate interrogatory to his attorney. (knowing this coverage may help in securing a new firm)

Can anyone provide a sample?

Sorry, I did not explain. The first attorney will receive nothing as he is voluntarily giving up the case and recommended other attorneys. We are also interviewing ones of our own choosing. The second attorney's fees will be negotiated based on much of the work being completed. (Unless of course the case goes to trial.)

Thanks. I found similar interrogatories with questions but was unsure if the same format would be used to request a copy of ones insurance policy.

Considering that question 4 asks specifically about the insurance coverages... I'm not sure why you would be unsure.

You are correct, thanks. #4 seems to be the language I need. I had not read the entire interrrogatory before I responded.
