The company I used to work for still owes my last paycheck. They have not filed for bankruptcy yet. I went to the labor board and they decided they owed me my back pay plus interest. They were however, unable to deliver their decision - sending me to file a suit. My claim is for more that 5k so I have to file a civile suit. My challenge - which form do I choose? I just want the amount the labor board decided on. I was given a list of about 150 forms from which I can choose, however they are not allowed to give any advice - so I'm stuck and time is ticking. Can anyone tell me which form to use? Im am in Los Angeles, CA.
