What is the name of your state? NY

Here is the story: I had a medical condition and needed surgery last year, then after the biospsy result came out, my doc told me to call the hospital counciler for the result. I did and that office in the hospital made an appt for me to see the counciler. Then I did. A few months later, I got a bill. My insurance refused to pay what they are supposed to cover claiming that when the consultation office billed them they did not put a referal number on. So it's my responsibility to pay for a specialist which is not referred by my doctor.

I know some will say "it's your responsibility to get a referal". What the ???? I did not call the conciler myself, in fact I didnt' even know they exist. I was told to go in and see her !
