At my institution, we require students (18-22) and their parents to sign a general and medical waiver for certain activities. The current practice is for a student and parent(s) to sign the same hard copy form and return it to the appropriate office. We are considering converting this to a "live" electronic form that can be filled out and submitted online by each party. The proposed mechanism is for the student to provide the parent's email address, and the system would send an encoded link to that student's form to the provided email address. The parent would then visit the link, complete the parent portion, and submit the form.

The concern is whether or not the involved chain of custody and the resulting electronic signature are rigorous enough to provide a valid, signed release. Any thoughts?

See an attorney as it depends on the facts like, what is your state and what is the state of the "signer". You will also have to review your specific risk. Google also: Electronic signature.

Also ... why do you share information about an adult with other adults? Do you have a waiver from the adult students to share such information?
