is it lawful for an employer to say you can only use the bathroom twice per shift and only for 5 minutes per visit and must obtain permission first before you can go????(sounds more like kindergarten)

For better or worse an employer is authorized to make such rules for its employees.

Sorry to hear about your situation, some employers are real control freaks. I've been in the same situation. I suggest either get a doctor's note saying you must have access to the bathroom or quit your job. Don't put up with that kind of abuse of power.
