What is the name of your state (only U.S. law)? Indiana

I asked before about how I could get a review of the current custody situation and no one replied. If I cannot get a review then I will file a petition for custody. My daughter's grades are falling somewhat. She also has cavities in her permantent teeth. She never had a cavity before.

My question is where I can find guidelines for filing pro se in Indiana. I now that everything need to be written in the proper format. I know that I need to file a copy with the court, but do I send a copy to my ex's attorney or will the court do that.

What about witnesses? should I call my ex first so he can lie, and then the judge will hear the truth from the guidance counselor, teacher, etc. What about my daughter's stepmom? Should I call her as a witness? She wrote notes to my daughter that she signed "Mom." Should I present those to her or my ex first? Should I question her before him? I need to plan my strategy to show the judge the true person behind my ex.
