Word to the wise. Create a document that has the notary space and get it notarized.

And make sure that whoever you give it to knows to TELL the medical staff repeatedly that there is one.

If you elect DNR (Do not resuscitate) and you're coding, the doctors aren't going to stop to look at file.

Ok I agree with that I think I will make sure it is a notarized copy. 1 last question do you now if the hospital requires the original or will a copy suffice?

It is best to get several notarized copies when you get the notarization done.
