Power Of Attorney

What is the name of your state (only U.S. law)? OH

I'm having a lot of trouble finding a POA online that I can use as a template. I found this one that applies to grandparents and I was wondering if I could use it and just replace the word grandparent. I just need to be able to take the child to doctors, WIC, and get services through Help Me Grow for him.

What child is it you need POA over?
My nephews baby brother. He lives with me, but I don't have custody. I can take him to his pediatrician, and in fact am the only person who has ever done so, but he needs to see other doctors sometimes and might have to have a sleep study done. I have always done his WIC also, but have to have his mom sign a paper every time and she is not always easy to track down. I pretty much just keep up with her on facebook to see where she is and what she is up to.
 
Why not get guardianship?
I didn't know I could. Someone had told me once that guardianship was for grandparents. What is the difference? Does it give me the same rights? Some of the things I've read state that the POA can only last for up to 6 mo. Could Gaurdianship last longer?
 
I think I found a good form, but what do I put here in the blank?

(e) I do authorize my/our aforesaid Attorney-in-Fact to execute, acknowledge and deliver any instrument under seal or otherwise, and to do all things necessary to carry out the intent hereof, hereby granting unto said Attorney-in-Fact full power and authority to act in and concerning the premises as fully and effectually as the Natural Parent(s) may do if personally present, limited, however, to the purpose for which this limited power of attorney is executed. The Attorney-in-Fact may execute any and all such documents or other papers in the following form: “______________________, Attorney-in-Fact for {name applicable Child}, a Minor Child”.

The Attorney-in-Fact may execute any and all such documents or other papers in the following form: “______________________, Attorney-in-Fact for {name applicable Child}, a Minor Child”.
What form is this because the grandparent POA is ONLY for grandparents. You could have someone write a POA that grants you permission and have it notarized but it would not be a court document.

I could get guardianship of your nephew if his parents agreed, you do not have to be a grandparent.

I would suggest that you get a meeting with a local attorney about getting guardianship. Right now, mom or dad can show up, take the child and leave, you have no rights and a POA will not give you any rights to stop them.

Guardianship, issued by the court, is easy enough if mom and dad agree. It does not remove their parental rights but it does give you some legal protections and allows you to seek care/education/etc for the child.

That would be custody in Ohio. Not guardianship. Ohio does not have such guardianship.
