What is the name of your state? Florida

My father is disabled and has a language barrier. He's Vietnamese and currently residing in a nursing home. His case manager requires his family to become his power of attorney. And he also cannot write his name too but he can only initial his first and last name. But the only problem is that I don't think he has the knowledge of knowing about it and he doesn't understand the system. So basically what I'm saying is that is it possible to get some sort of POA service to do it or is it illegal to do this without his permission even though it is for his well being to have his family take care of his medical stuff.

Zig is correct. If he is unable to understand the power of attorney document (even with appropriate translation and explanation), he can't execute it. If he's competent to make other legal decisions, chances are you're not getting a guardianship, but it sounds like that he's probably beyond that. What the "case manager" wants is absolutely irrelevant.
