What is the name of your state (only U.S. law)? OHIO

Hi, I'm in the middle of trying to go through a "deed in Lieu" with my mortgage company. They are saying the title is not clear, and I must clear it to go forward with this. The reason... Because the original deed from 2002 was written incorrectly. I purchased my home from a married couple at the time. The deed states that each of them were married. (basically saying they were both married to other people) And according to what another lawyer told me, this still entitles them 'interest in the deed'.

I am in contact with this couple, and they are willing to sign whatever they need to help me out with clearing this title, and getting them off the deed.

My question is... What exactly do I need to get this done? A simple affidavit? If so, can anyone help me on how I can write this myself?

Also, apparently I was young and dumb, and did not get myself covered under title insurance when I made the home purchase. :(

Any help with this, is greatly appreciated!!!
