health insurance cancelled without explanation, but with reimbursement of paid premium by one company, then denied (by another company) to 33-yr-old man because of handwritten clinical notes taken when he was 16, in another state (above), and a dependent of his parents

what's to be done?

I am not sure anything can be done as the underwriting decision does not seem arbitrary or unreasonable if based on material in the file that appears accurate. I know of no "statute of limitations" on use of adverse medical info, but ask your state insurance department.

It seems unconscionable to use remarks made to a psychiatrist when he was 16 to deny health coverage to a man of 33 with no intervening mental health issues.
How could this information even be released without his parents' permission?
