What is the name of your state? Colorado

Is it possible to add the father to a birth certificate just by signing an AOP if the parties are unmarried and the child is almost 2?

yes, or at least you can in my state. The AOP needs to be notarized and filed with the bureau of vital statistics. This will NOT change your childs name however, you need a court order for that.

But if the fater and mother both want to give the child the father's last name, then why would it need to be court ordered? Is it because the AOP is signed years after the childs birth instead of right after the childs birth?

This is not true in every state. Some states allow a change of name by affidavit of the mother and father.

OK....So if you can add the father to the birth certificate by having him sign an AOP; and you can correct birth certificates; is it possible to correct the birth certificate and add the fathers last name by hyphenating the last name that is already listed on the birth certificate?
